Communities for Healthy Aging Transitions

CHAT, or Communities for Healthy Aging Transition, is a regional project to look at age-friendliness in Strafford County, as well as Brookfield, Wakefield, Newmarket, Nottingham and Northwood in Carroll and Rockingham counties, respectively. It is funded by TUFTS Health Plan and is a partnership with Greater Seacoast Community Health. 

Informed by the CHAT steering committee, the mission of the project is to engage these to foster diverse, equitable, inclusive, livable environments where people can grow and thrive, regardless of age. 

The steering committee consists of a group of diverse partners that represent community entities like UNH, the Community Action Partnership of Strafford County, ServiceLink, Seacoast Outright, and others.

Outreach events related to the project are being scheduled for 2021, which include community input sessions, a region-wide online survey, focus groups, an interactive art activity, and more.

Project outcomes include a community assessment report for each of the 18 communities in the region, four action plans and two subsequent pilot projects for interested municipalities, and a comprehensive research guide.
